


The role of the case manager will vary according to individual needs and circumstances, but may include:-
- Full and comprehensive assessment of current abilities and needs, including physical, cognitive, psycho-social and functional.
- Assisting client and family to develop a realistic life plan, identifying immediate and longer term goals such as to live in their own home, or to return to some form of employment.
- Establishing rehabilitation potential and, when appropriate, to source relevant neuro-rehabilitation services. This may be in a formal centre or by developing a "home based" programme.
- Acting as an advocate for the individual and providing support to the family.
- Undertaking care and support needs assessments and developing bespoke care packages to meet individual needs.
- Recruiting, training and managing complex community care packages on an ongoing basis.
- Providing support to vulnerable clients who live alone.
- Advising on housing, equipment and further support services, and where appropriate enrolling the help of other professionals who have specialist knowledge in these relevant areas.
- Practical problem solving.
