This will depend on the needs of the client and the particular family circumstances. The most dependent and complex clients will benefit from case management involvement for life, although the level of intervention will rise and fall at different stages in their life.
Other clients will require active case management involvement for 2 or 3 years and possibly again at times of major life change, but will manage without professional involvement once services are well established, or they have reached a level of independence and no longer require regular support.
