Referrals can be accepted from clients or families themselves, solicitors, rehabilitation services, Receivers and the statutory health and social services.
On receipt of a new enquiry we will identify that the client is likely to benefit from our services and then provide a quote for an initial assessment visit and report.
The initial visit lasts 2-3 hours, will normally take place where the client is currently living, and includes a meeting with key family members. This gives the individual a chance to meet a case manager and ascertain whether they want them to work with them. As the relationship between theindividuals, their family and the case manager is paramount to providing an effective service, it is important that the family/client feel they have some choice and a decision to proceed is not based solely on the fundholder (lawyer or statutory provider).
Following the visit a short report is prepared setting out:-
- The current situation
- Immediate case management goals
- How much input is envisaged as necessary over the next 1-2 year period
- An estimate of likely costs
On receipt of this report the family and fundholder should confer in order to agree that they wish to appoint Anglia Case Management Ltd. to provide ongoing clinical services.
