Sarah is an Assistant Case Manager and has her own case load whilst working alongside the lead Case Manager for each client.  This includes:

  • Providing day to day case management to clients whose cases have settled, under the supervision of a lead Case Manager;
  • Management of support teams; including the supervision of staff, assisting with care planning and risk management, ensuring compliance with CQC requirements;
  • Assisting with the sourcing of information, services and specific equipment needs for clients;
  • Monitoring of rehabilitation programmes and supporting clients to develop structured and purposeful activities.

Sarah has worked in the Health and Social Care sector for the last 12 years in a variety of roles.  She has experience of managing cases with adults and young children who have complex needs, in their own home, and this has included managing and supervising the support team.  She works alongside the Case Managers in the recruitment and training of staff to work with clients in their own homes; she is skilled in working with support staff and other rehabilitation professionals to ensure the safe management of these clients.

Sarah has a good understanding of the CQC process and its requirements, so can work with support teams to ensure they are compliant. She has excellent communication skills and has wide ranging experience of liaising with professionals from the statutory and independent sectors.

Sarah uses the experience she gained as a rehabilitation assistant, where she was working alongside the multi-disciplinary team in a hospital setting supporting clients to realise their potential and return home, to good effect when working with and supporting ACM clients to live independently in the community.

Sarah is employed full time; she is office based and has access to full administrative support, resources and professional supervision from senior Case Managers.
