Julie ChorltonCase Manager

Julie is a qualified nurse who brings to the Case Manager role extensive experience of working in the NHS, business environments and in clients’ own homes.  Julie has undertaken complex client assessments, and provided the care and support that her clients have needed, working professionally to effect the best possible outcome.

Julie’s experience includes supporting clients with complex needs following brain injury and also those with spinal injuries, home ventilation and tracheostomy management.  She has been responsible for recruiting and training those staff in the care required ensuring compliance for CQC standards.  Julie also has case management experience with amputee clients working within a multi-disciplinary environment to effect a good outcome for the client.

Julie has managed others, led by example and provided the co-ordination to ensure the team works in the best interests of the client.  She has excellent clinical and communication skills, she is a highly organised and experienced field worker.
