Stephanie Graham, CQSW

Case Manager

Stephanie has over 25 years’ experience working with disabled children and young people, and their families.  Her knowledge and experience has been accrued from working in the Voluntary Sector with physically disabled children and young adults; through a Local Authority’s Disabled Children’s Service; and more recently in the charitable sector as a Care Manager within a Children’s Hospice.  Stephanie has particular experience in working with children with the most complex of needs, including all types of Cerebral Palsy and life-limiting conditions.

Stephanie is a Social Worker and has held myriad roles relating to disabled children with complex health needs during her career.  She has been a case worker and managed both teams and services.  She has experience of working in a hospital setting within a multidisciplinary team carrying out assessments and supporting families at the point of diagnosis; through to holding the ‘Lead’ role for transitions for the Local Authority across Children and Adult services.

In 2012 Stephanie was employed with a Children’s Hospice where she managed a multidisciplinary team providing therapeutic services to children receiving palliative care and their families.  She has experience of working with families living with high levels of distress – preparing for and living with bereavement.  Stephanie held the Safeguarding supervisor role within the Hospice, and was the ‘go to’ person for staff having safeguarding concerns relating to children and their families.  She also delivered training for all Hospice staff relating to person-centred approaches and is very passionate about this way of working with clients.

During her career she has worked closely with education and health professionals, at both case work and service development levels.  She has worked with parents to develop their knowledge of person-centred tools and to help give them the courage to advocate on behalf of their children and young people. She has extensive experience in the co-ordination of professionals and services, staff recruitment and line management.
