Assistant Case Manager
Claire has 14 years’ experience working in Social Care sector and joined Anglia Case Management as an Assistant Case Manager. As an Assistant Case Manager, Claire works alongside an overseeing professionally qualified Case Manager. Claire’s involvement with clients is varied depending on the needs of the client; the majority are adult cases however, she can also work with young people.
Claire’s main role at ACM is to manage a small caseload of clients whose personal injury litigation has settled and they require long term case management. The role encompasses the everyday needs of the client, phone support with problem solving, physical support to attend meetings and appointments and ensuring the client’s welfare and rehabilitation needs are met. Claire will liaise with the client’s Financial Deputy to secure funding for various activities and items required by the support team in order to carry out their work with the client. She also she undertakes the line management of Support Workers and Team Leaders, undertaking ongoing supervision, appraisal and training. Claire also ensures that packages of care and support meet the needs of the client and comply with CQC regulations.
In addition to the longer term pieces of work she also supports Case Managers at ACM by undertaking short term tasks, including:
- Holiday planning where the client requires support;
- Accommodation searches and support with moving taking into consideration all the factors associated with moving including care needs;
- Attending MDT’s and providing relevant information;
- Therapist searches.
Prior to joining ACM, Claire had previously worked for Social Care in Essex and had gained experience working in both the community and hospital settings. Her client group in her role with Essex Social Care was varied but included working with older people, and adults with learning disability and/or physical impairments. Claire has experience of managing a case load, liaising with professionals, supporting adults with housing adaptations, researching housing options and supporting adults to move, seeking employment opportunities and supporting adults in their daily life and helping them to achieve their goals.
