ACM has been involved in my sister’s care since her accident in 1988. Our assigned case managers have helped Helen through some particularly difficult times, such as a safeguarding issue at her previous care home and becoming critically unwell. Input from ACM over the years has held the care providers accountable in a way that would be very difficult for family members not familiar with the industry. I cannot stress enough how significant this input has been. Our current case manager has a very high level of knowledge. In the past year, their Case Manager has been instrumental in reinstating funding which allows Helen one 4-hour outing a week with 2 carers. Recently she has facilitated improved engagement with Helen’s home staff to improve physio sessions, daily exercise bike use and the ensuring the actioning of planned activities. She works to ensure rehabilitation, social and care activities occur and are appropriate. Helen’s case management is tailor-made for her, and we cannot fault it.
